Community Health Worker - AmeriCorps

We're currently seeking candidates in a variety of New England locations to connect individuals and families living with epilepsy to the social and community resources they need to navigate the challenges of everyday life and to improve their healthcare outcomes. Full-time positions offer paid service training leading to certification as a Community Health Worker. As a Community Health Worker, you'll be a valuable conduit for patients seeking information and support.

Opportunities will include working with a talented team at EFNE and one of several major medical institutions, providing referrals to a wide variety of community-based programs and resources to increase the quality of life for patients and families living with epilepsy. With flexible hours and a hybrid work environment, you'll have the opportunity to make a real impact in the lives of those living with epilepsy. This 11-month service program begins as early as January 2024, but recruiting will continue until all positions are filled.

Responsibilities include:
  • Provide active listening, support, and coaching to patients to improve health and wellness
  • Build connections among patients through community-based events
  • Connect patients to EFNE, hospitals, and community resources
  • Arrange inpatient support and/or recreational programs
  • Assist in new initiatives as well as participate in interventions, telehealth, resource referrals, and back-to-school support
  • Mentor patients and connect them to community resources
  • Conduct community outreach
  • Maintain a library of helpful literature
  • Provide support to hospitals with their unique needs and programming
Requirements:
  • High School Diploma/GED
  • 18 years of age or older
  • Legal authorization to work in the United States
  • Strong communication skills
  • Passion for working with diverse populations
  • Motivation and aptitude for outreach and networking
  • Interest in nonprofit organizations and their development
  • Ability to balance AmeriCorps service with other commitments in a sustainable way
  • Organizational and communication skills
  • Proven project management abilities
  • Ability to work independently and as part of a team
  • Flexibility, patience, and a good sense of humor

Service commitment will average 35 hours per week, generally performed Monday-Friday 9 am-5 pm, with some weekends and evenings scheduled with notice.
